QUICKmed Urgent Care is seeking a compassionate and detail-oriented Health Services Coordinator to support key clinical initiatives focused on patient care and value-based performance. This role is responsible for managing chronic care programs, quality incentive initiatives, vaccine coordination (including the Vaccines for Children Program), and supporting behavioral health services. The coordinator will act as a vital liaison between patients, providers, and health partners to ensure high-quality, coordinated care.
Care Program Management
- Lead the Chronic Care Management (CCM) program, including patient enrollment campaigns, provider collaboration, care coordination activities, and productivity reporting through CCIQ.
- Manage incoming care management referrals and ensure timely patient outreach and care planning.
- Oversee Optum and Ohio Medicaid Quality Incentive Programs by:
Monitoring trends and incentive opportunities.
Training team members on proper visit note documentation.
Communicating with providers about opportunities to close care gaps.
- Conduct annual reviews of Ohio Medicaid’s Comprehensive Primary Care program requirements and update policies and procedures accordingly.
- Present program materials to IPRO for compliance and program sustainability.
Population Health & Quality Improvement
- Design and implement population health strategies, including:
No-show reduction initiatives.
Chlamydia and cervical cancer screening improvement projects.
Wellness exam campaigns.
Voice of the patient surveys.
- Collaborate with Medicaid MCOs and the Director of Operations to implement provider side interventions aimed at improving health outcomes.
- Analyze attribution data from the ODM PNM system and ensure proper payments are received.
Vaccine Administration
Oversee vaccine ordering, inventory control, and compliance with CDC and VFC program requirements.
- Ensure proper cold chain storage and documentation.
- Train staff on vaccine handling protocols and monitor vaccine usage and expiration.
- Coordinate with public health agencies and vendors for seamless vaccine delivery and compliance.
Care Coordination & Patient Engagement
Support transitions of care and follow-up for discharged patients.
- Facilitate patient education, outreach, and communication between providers and patients.
- Act as back-up to the Licensed Social Worker for mental health scheduling, coordination, and documentation.
Administrative & Operational Support
Maintain accurate and up-to-date records in the EHR, ensuring data integrity across systems.
- Contribute to cross-functional meetings and participate in organizational quality improvement projects.
- Perform additional duties as assigned to support clinical and operational excellence across locations.
Qualifications Required:
- Knowledge of chronic disease management, care coordination, and immunization best practices.
- Strong organizational, interpersonal, and communication skills.
- Proficiency with EHR systems and Microsoft Office.
Preferred:
Clinical background (MA, LPN, or equivalent).
- Experience with value-based care (VBC), CCM, and quality measure tracking (e.g., HEDIS, CMS Star Ratings).
- Experience in vaccine coordination and behavioral health support.
- Familiarity with Medicaid programs, reporting tools, and provider performance data.
