Job Overview
We are seeking a Nurse Practitioner or Physician Assistant to join our team. We are a privately owned and operated Family Practice office. Our mission is to provide the utmost care to our patients. We believe in continuity of care, as most patients are seen every 3 months and each provider is very familiar with their patients and their needs. We aim to achieve the best health outcomes and ensure our patients feel they come first.
The ideal candidate is compassionate and has a strong understanding of chronic medical conditions. One is expected to provide high-quality patient care, perform comprehensive assessments, and develop treatment plans in collaboration with our physician.
This role requires excellent communication skills, and the ability to work independently in a fast-paced environment.
Responsibilities
- Perform annual physicals, patient assessments and obtain and document medical histories.
- Provide preventative care and educate patients regarding their conditions and the importance of maintaining health.
- Manage complex medical conditions typically encountered in Family Practice out-patient setting.
- Able to treat acute care illness and develop a treatment plan to achieve wellness.
- Order and interpret lab and radiology testing and explain findings to patients.
- Excellent interpersonal skills with the ability to communicate effectively with patients and their family members.
- Utilize electronic health records systems (EClinical) for documentation and patient management.
- Collaborate with neighborhood specialist teams to coordinate care for patients.
- Ensure compliance with HIPAA regulations to protect patient confidentiality.
Requirements
- Physician Assistant or Nurse Practitioner degree from an accredited program.
- Current certification and unrestricted NY state licensure.
- Experience in acute care settings preferred, particularly in outpatient environments.
- Utilize ICD-10 coding for accurate diagnosis documentation.
- Ensure compliance with HIPAA regulations to protect patient confidentiality.

Pay: $110,000.00 - $125,000.00 per year
