#### Job Overview:We are seeking a qualified and dedicated Nurse Practitioner (NP) to join our interdisciplinary team at WVVNF. The NP will be responsible for providing clinical evaluations, managing patient care programs, and collaborating with healthcare professionals to ensure the highest quality of care for residents. This position involves direct communication with physicians, staff, residents, and families, as well as completing necessary documentation in the electronic medical records system.### Responsibilities:- Perform clinical evaluations, treatment programs, and prescribe appropriate medical orders.- Write medical histories, design treatment programs, and handle discharges, referrals, and care records.- Make daily rounds to assess resident care and collaborate with healthcare providers.- Collaborate with dieticians, wound care nurses, pharmacy consultants, and other health care providers for treatment and medication management.- Communicate with attending physicians, directors of nursing, residents, families, and outside healthcare providers.- Complete referrals for specialized medical services and ensure the continuity of care for residents.- Attend interdisciplinary team meetings and contribute to discussions on resident care concerns.- Review and maintain medical documentation, including progress notes, treatment orders, and assessments.- Respond to calls during non-working hours as required, even on days off. ### Minimum Qualifications:- **WV Registered Nurse Practitioner License**: Must hold a valid license in the state of West Virginia.- **Long-Term Care Experience**: A minimum of 3 years of experience working in long-term care settings is required.- **Federal and State Regulations**: Must have a thorough understanding of Federal and WV State long-term care regulations.- **CPR Certification**: Must maintain a current Cardiopulmonary Resuscitation (CPR) certification. ### Mandatory Requirements:- Successfully complete a background check through WV Cares and a twelve-panel drug screening before placement.- Must provide resumes and supporting documentation for selection. The agency will interview NP candidates prior to placement.- Maintain professional communication with the healthcare team, family members, and external healthcare facilities.- Must be available by phone 24/7 for any urgent resident care needs during off-hours (calls expected to be minimal).### Additional Expectations:- The NP must ensure continuity of care even when referring residents to other specialists.- Although the NP will not manage the nursing department, they will collaborate closely with the team for optimal resident care.- Must attend regular meetings on care issues and contribute to interdisciplinary discussions regarding residents' health and well-being. ---This role is crucial for maintaining the health, safety, and comfort of our residents.
