Job description
Qualification Requirements All Temporary/Relief Physician Assistants and Nurse Practitioners assigned to Correctional Facilities must have the minimum experience outlined below:
Nurse Practitioner Requirements
1. Two (2) years of experience in primary care (adult) as a Nurse Practitioner and within the last three (3) years rendering direct patient care in public or private institution/facility. Clinical competencies must be current and validated for the area in which Temporary/Relief Nurse Practitioner will be assigned.
2. Possess and maintain while performing services for the Correctional Facilities a current and valid license issued by the California Board of Registered Nursing to practice as a Nurse Practitioner in the State of California including adherence to all laws and regulations governing the practice of Physician Assistant in California. In addition, complete thirty (30) hours of continuing education every two years Courses must be taken through a continuing education provider recognized by the Board of Registered Nursing (BRN).
Physician Assistant Requirements
1. Two (2) years in Family Medicine/Family Practice (adult) as a Physician Assistant and within the last three (3) years rendering direct patient care in a public or private institution/facility. Clinical competencies must be current and validated for the area in which Temporary/Relief Physician Assistant will be assigned.
2. Possess and maintain while performing services for the Correctional Facilities a current and valid license issued by the Physician Assistant Board to practice as a Physician Assistant in the State of California including adherence to all laws and regulations governing the practice of Physician Assistant in California. At license renewal, complete 50 hours of medical education every two years or by obtaining certification by the National Commission on Certification of Physician Assistants.
Specialty Services
1. In addition to the above requirements, Temporary/Relief Physician Assistants and Nurse Practitioners who are requested to work in a specialized area such as emergency medicine, surgery, etc., shall also have at least two (2) years of experience within the last three (3) years providing patient care within that setting and possess any required certificates
Obstetrics/Gynecology
2. Temporary/Relief Physician Assistants and Nurse Practitioners providing services to female Institutions/Facilities must also have at least one (1) year of experience within the last five (5) years rendering obstetric and gynecological nursing or patient care services in a healthcare setting in addition to the above qualifications. Clinical competencies must be current and validated for the area in which staff will be assigned.
3. Under Business and Professions Code 2072, out-of-state licensed physicians can be appointed to California state institutions for up to two years, under the supervision of a California-licensed physician. They must obtain a California medical license within that time to continue practicing. This code does not apply to physicians already licensed in California. 4.Temporary/Relief Physician Assistants and Nurse Practitioners must be proficient in English to effectively communicate with Correctional Facilities/CCHCS. They must be capable of speaking, understanding, and writing in English. Those who do not meet these language requirements will not be allowed to provide services.
Service Delivery
Requirements Under the direction of the Institution/Facility CEO/CME or designee, the Temporary/Relief Physician Assistant and Nurse Practitioner shall provide services as permitted within the scope of practice for Physician Assistant and Nurse Practitioner services in accordance with Institution/Facility policies and procedures.
A. Standards of Obligation
Temporary/Relief Physician Assistants and Nurse Practitioners must provide high-quality care according to accepted medical standards to support Correctional Facilities/CCHCS in managing healthcare services for California's patients and youth. They must follow Correctional Facilities/CCHCS rules, policies, and infection control measures, with the Agreement not altering common law standards of medical practice.
B. Authorization 1. Authorization for Treatment Temporary/Relief Physician Assistants and Nurse Practitioners must obtain written prior authorization for non-emergency services from the Correctional Facilities/CCHCS CEO/CME or designee, document it in the patient's record, and complete all required forms, as per CCHCS's Utilization Management Pla
2. Authorization for Transportation/Transfers Temporary/Relief Physician Assistants and Nurse Practitioners shall agree that, as required by law, Correctional Facilities /CCHCS retains full authority to determine the manner in which a patient/youth is transported to the Correctional Facilities /CCHCS institutions/facilities or transferred to other healthcare facilities, after the course of treatment or therapy has been implemented or completed.Temporary/Relief Physician Assistants and Nurse Practitioners shall not transfer a patient/youth to any facility without prior written authorization from the appropriate Correctional Facilities/CCHCS Institution/Facility CEO/CME or designee
3. Experimental and Investigational Drugs and Procedures Temporary/Relief Physician Assistants and Nurse Practitioners cannot administer experimental treatments without prior approval from the Institution/Facility CEO/CME and Correctional Facilities Office of Legal Affairs. They are only allowed to provide services that are safe, effective, and meet accepted medical standards.
4. Exclusions and Limitations
a. Temporary/Relief Physician Assistants and Nurse Practitioners must agree not to provide healthcare services or treatments for conditions specifically excluded under CCR, Title 15, Division 3, Chapter 1, Subchapter 4, Article 8, Section 3350.1. However, Physicians, Physician Assistants, and Nurse Practitioners can request exceptions for these excluded or limited services by obtaining prior approval from the Institution/Facility CEO/CME or their designee.
b. Temporary/Relief Physician Assistants and Nurse Practitioners can provide excluded services for pre-existing conditions only if the condition severely worsens and threatens the patient’s health. Any appeal for such services must be considered medically necessary.
c. The Correctional Facilities/CCHCS Medical Standards of Care shall be used as a reference when prior authorization is granted.
5. Clinical Reviews
Temporary/Relief Physician Assistants and Nurse Practitioners may be asked to perform clinical reviews to assess care quality, including reviewing adverse outcomes and participating in Professional Practice Committee activities. This may involve examining records, discussing cases, and conducting patient examinations. They must be skilled in preparing written review documents.
6. CCHCS will provide the Correctional Facilities formulary to Temporary/Relief Physician Assistants and Nurse Practitioners, who must consult Correctional Facilities physicians to prescribe only formulary medications unless prior authorization for exceptions is obtained.
