Job Description: Nurse Practitioner (NP)
Position: Nurse Practitioner (NP)
Location: West Virginia
Work Hours: 8:00 AM to 4:00 PM, 3 days per week (to be determined by the Facility Administrator)
Qualifications:
1. Licensure:
· Must hold a valid West Virginia Nurse Practitioner (NP) License.
· Must possess and maintain a valid DEA License to write prescriptions
2. Experience:
· Minimum of 3 years of Long-Term Care experience is required.
3. CPR Certification:
· Must possess and maintain a current Cardiopulmonary Resuscitation (CPR) certification.
4. Regulatory Knowledge:
· Must be knowledgeable of and abide by Federal and State Long-Term Care regulations.
Responsibilities:
1. Clinical Care:
· Perform clinical evaluations, develop treatment programs, and collaborate with other healthcare providers to ensure comprehensive care for residents.
2. Documentation:
· Write medical histories, treatment plans, care records, and discharge documentation.
· Review charts for quality and accuracy in medical documentation.
3. Prescribing and Orders:
· Prescribe medications, including maintenance drugs, narcotics, and Schedule II opioid drugs.
· Prescribe orders for laboratory and diagnostic tests.
· Access and utilize the WV Controlled Substance Monitoring Program Database as required by law.
4. Collaboration:
· Collaborate with the Dietician/Dietary Manager for managing weight changes and dietary plans.
· Work with the Wound Care Nurse for the treatment of ulcers and other wound care needs.
· Collaborate with the Pharmacy Consultant for medication management.
5. Resident Rounds:
· Make daily rounds to residents to monitor their care and ensure that health care plans are being properly executed.
Mandatory Requirements:
1. Drug Screening:
· Must pass a drug screening and will be subject to random screenings at any time. Vendor covers the associated cost.
2. Criminal Background Check:
· Must pass a criminal background check through WV Cares and will be subject to random checks during the contract period. Vendor covers the associated cost.
3. Timely Candidate Provision:
· Vendor must provide valid NP candidates within 10 business days of Agency's request. Failure to do so may result in default of contract.
4. Supporting Documentation:
· Vendor must provide resumes, licenses, and other supporting documentation for each candidate.
5. Replacement Provision:
· In case of departure, Vendor must provide a replacement NP within two weeks. Failure to provide a replacement within this time frame may result in contract default.
6. Work Consistency:
· Agency prefers the same NP to be retained throughout the contract duration, but understands that this may not always be possible.
Work Hours and Holidays:
· The NP will work 8:00 AM to 4:00 PM for three (3) days per week, with the exact days determined by the Facility Administrator.
· Official West Virginia State holidays may be days off, and no payment will be made for time off.
State Holidays include: New Year’s Day, Martin Luther King Day, Presidents Day, Memorial Day, West Virginia Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, the Day after Thanksgiving, and Christmas Day.

Pay: $69.00 - $73.00 per hour
