Key Responsibilities:
- Personal Care: Provide high-quality nursing care to residents, including administering medications, monitoring vital signs, and assisting with daily activities.
- Care Planning: Collaborate with the healthcare team to develop and implement individualized care plans for residents.
- Documentation: Maintain accurate and timely documentation of resident care, observations, and any changes in health status.
- Communication: Communicate effectively with residents, families, and other members of the healthcare team to ensure continuity of care.
- Compliance: Adhere to all state and federal regulations, as well as facility policies and procedures, to ensure the highest standards of care.
- Support: Assist in training and mentoring new staff members and providing guidance as needed.
Qualifications:
- Education: Graduate of an accredited LPN program or willing to complete an LPN program
- Licensure: Current and valid LPN license in the state of Pennsylvania.
- Experience: Open to new graduates and experienced LPNs.
- Skills: Strong clinical skills, attention to detail, and the ability to work effectively in a team environment.
