As a Case Coordinator, youll play a key role in supporting clients, coordinating essential services, and advocating for their well-being. This role involves assessing clients' needs, developing and implementing individualized care plans, coordinating services, and advocating for clients' access to community resources.
- Bachelors degree in social work, psychology, counseling, or a related field.
- Minimum of 2 years of experience in addiction services, case management, or a related field. Experience working with individuals with substance use disorders preferred.
- Relevant licensure or certification preferred.
- Valid drivers license and reliable transportation for community-based work.
- Ability to work flexible hours, including evenings and weekends, as needed.
- CPR and First Aid certification preferred.
