The role of a Qualified Professional is essential in providing high-quality care and support to individuals with diverse needs. The Qualified Professional will be responsible for implementing care plans, monitoring patient progress, and ensuring a safe and supportive environment for clients, particularly those dealing with autism, dementia, and other health challenges.
Duties
- Deliver compassionate home health services tailored to the individual needs of clients.
- Provide assistance with daily living activities including meal preparation, personal hygiene, and mobility support.
- Administer medications as prescribed and monitor patients for any side effects or changes in condition.
- Conduct regular assessments of client health status and report any significant changes to the healthcare team.
- Ensure compliance with HIPAA regulations to protect patient privacy and confidentiality.
- Engage clients in meaningful activities that promote social interaction and cognitive stimulation.
- Support families by educating them on care techniques and available resources for senior care.
Skills
- Experience or training in working with individuals with intellectual developmental disabilities.
- Proficiency in mental health and home health practices including intellectual developmental disabilities, behavior disorders, and dementia care techniques.
- Strong understanding of patient care principles and best practices.
- Ability to prepare nutritious meals that cater to dietary restrictions or preferences.
- Knowledge of medication administration protocols and patient monitoring practices.
- Familiarity with HIPAA regulations to ensure patient confidentiality is maintained.
- Compassionate approach towards senior care, demonstrating patience and empathy.
- Strong understanding of Individual Service Plans (ISPs), and the ability to write Person Centered Plans (PCPs) and long- and short-term goals.
- First Aid, CPI certified
- Ability to effectively communicate with families, doctor's office, participants, staff, governing bodies, state entities, manage care organizations, such as Trillium, Alliance and Vaya.
