Purpose:
The purpose of the Nurse-LPN/RN position is to provide support to adult clients who receive care from the medication clinic by providing professional nursing care, care management and coordination services.
Accountabilities:
- Engage individuals and families as partners in services and develop positive, hopeful and trusting relationships.
- Assists individuals and families in avoiding and resolving crisis situations.
- Works closely to provide education and support to clients, families, or other support persons the client identifies.
- Provides care coordination with other treatment providers, both internally and externally.
- Supports the prescribing physician and/or nurse practioners as needed.
- Provides assistance as needed for scheduling within the medication clinic.
- Works with the Director of Nursing and administrative assistant to maintain an inventory of medical and office supplies for the medication clinic.
- Interacts with pharmaceutical representatives as needed to coordinate with other providers.
- Assists with making referrals for other services within Wyandot Center and other medical and psychiatric facilities in the community according to the prescribers order.
- Works with staff to prioritize urgent needs.
- Returns phone calls that come ot the clinic services to determine level of need and develop a plan for accessing care.
- Works with the pharmacy to coordinate prescription assistance plans.
- Administer medications to clients as needed including but not limited to administering injections and oral medications.
- Works closely with the Medication Assisted Treatment Providers to assess patients for opioid withdrawal, conduct urinalysis and other assessments as needed.
- Able to draw blood for labs including finger stick and venipuncture for both point-of-care testing and comprehensive lab analysis.
- Thoroughly and timely completes all documentation and reporting requirements.
- Demonstrates positive interactions with clients, other members of the agency and external community partners.
- Is familiar with and complies with all relevant policies and procedures.
- Performs other duties as deemed appropriate by immediate supervisor or designee.
Qualifications:
- Must by licensed in Kansas by the Kansas Board of Nursing as a Licensed Practical Nurse or a Registered Nurse.
- Familiarity with mental health theory, emerging promising practices and Evidence-Based practices.
- Prior experience with mental health is preferred.
- Knowledge of medical and non-medical services delivery system.
- Effective communication with clients, family and supports, guardians and other services providers.
- Ability to establish relationships with individuals and organizations of influence within the social services community.
- Demonstrated strong interpersonal and communication skills.
Work Conditions and Considerations
- This position will be based at the 47th Street Location and/or the 8th Street location
- Transportation from other Wyandot BHN locations or various community sites required to meet business needs of the clients.
