Blended Hourly W2 Pay Rate: $53.00
Blended Weekly Gross Pay Rate: $1,908.00
Minimum Hourly W2 Pay Rate: $19.00
Min. W2 Weekly Gross (Taxable): $684.00
Weekly Stipend (Non-Taxable): $1,224.00

For Traveler 1099 Candidates:
Pay Rate on 1099 to the Candidate: $55.00
Weekly 1099 Traveler Pay Rate: $1,980.00
Position Overview:
We are seeking a compassionate and experienced Admissions Care Coordinator - LPN for a 13-week contract in Lewiston, ME. Under the guidance of the Director of Nursing, the Care Coordinator will be responsible for assessing admission referrals, evaluating patients' care requirements, verifying benefits, and ensuring the alignment of patient placements with organizational goals. This position requires a combination of clinical skills, attention to detail, and strong communication abilities. The ideal candidate will have experience in geriatric care and demonstrate flexibility, customer service, and organizational skills.
Key Responsibilities:
- Analyze clinical and financial data to identify suitable patients for placement according to organizational strategies.
- Provide prompt responses to referring facilities after assessing the appropriateness of patient placements.
- Gather necessary medical, financial, and social information to establish a complete care plan for patients.
- Disseminate admission information to the multidisciplinary team and consult medical teams for referrals as needed.
- Collaborate with patients and their families to establish clear admission goals and care plans.
- Develop, review, and regularly update individualized care plans, incorporating age-specific care strategies.
- Review and document discharge plans/instructions with patients, making referrals to resources as necessary.
- Conduct follow-up calls with patients to ensure satisfaction and quality of care, administering satisfaction surveys as needed.
- Act as a liaison between external agencies and the interdisciplinary team to facilitate successful patient admissions.
- Ensure patient readiness upon admission, including verifying medications, IV therapy, and equipment needs.
- Familiarize with acceptable diagnoses for admissions and reimbursement processes.
- Maintain patient safety through infection control and proper equipment use, adhering to clinical standards.
- Manage crisis situations calmly, coordinating necessary discharge needs with all relevant disciplines.
- Verify managed care company rates and insurance coverage, securing pre-certification for necessary equipment.
- Complete referral documentation and prepare necessary patient information for other departments.
- Document nursing care actions, observations, and responses, reporting changes promptly and accurately.
- Attend department meetings, offering education and serving as a resource for staff.
- Stay current with field advancements, maintaining education and training logs.
- Follow safety and infection control practices, attending safety education sessions to promote safe practices.
- Support and uphold the mission and values of Covenant Health Ministry.
Qualifications:
- Experience: Minimum of 3 years of experience in a similar role, ideally in geriatric care.
- Certification:
- Valid State of Maine Licensed Practical Nurse (LPN) certification.
- Current BCLS certification.
- Skills:
- Strong communication, customer service, and organizational skills.
- Ability to analyze clinical and financial data.
- Strong multitasking ability and decision-making under pressure.
- Knowledge of medical terminology and geriatrics is desired.
- Ability to work independently and as part of a team.
- Strong computer skills and understanding of software systems.
- Other Requirements:
- Flexibility to provide coverage as needed, including potentially floating to other units.
- Ability to speak, read, and understand instructions in English.
Schedule and Compensation:
- Shift Hours: Day shifts, with flexibility as needed.
- Guaranteed Hours: 36 hours
- Contract Length: 13 weeks.
- Shift Days: Monday to Friday (flexibility required).
