Job Title: Licensed Practical Nurse - LTC

Location: Tomichi, Gunnison, CO 81230

Duration: 13-Week Contract (With possibility of extension)
Shift: Days, 12-hour shifts

Guaranteed Hours: 36

OT: After 40 hours

OT Rate: All-inclusive Rate $10

Pay Rate:

For Local Candidates pay rate: $42 to $45/hr. (If your permanent address is within 50-miles from the facility)

For Travelers/Stipend Candidates (If your permanent address is outside the 50-mile radius from the facility):
Blended Hourly W2 Pay Rate: $45.00
Blended Weekly Gross Pay Rate:  $1,620.00
Minimum Hourly W2 Pay Rate: $15.00
Min. W2 Weekly Gross (Taxable): $540.00
Weekly Stipend (Non-Taxable): $1,080.00

OR

For Traveler 1099 Candidates:
Pay Rate on 1099 to the Candidate: $47.00
Weekly 1099 Traveler Pay Rate:        $1,692.00

Job Description:

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. The primary responsibility of this position is to assist in the supervision and coordination of daily nursing activities during your assigned shift, ensuring that high-quality, person-centered care is delivered in accordance with all Federal, State, and local standards, as well as facility guidelines. The LPN will work under the direction of the Director of Nursing and contribute to maintaining a safe, nurturing, and respectful environment for all residents.

Licenses:

• BLS Certification.
• LPN License for the State of Colorado/Compact.

Skills required:

• Supervise and coordinate the delivery of nursing care to residents during assigned shifts.
• Assist in developing and implementing nursing care plans and ensure proper documentation.
• Administer medications and treatments as prescribed, following proper protocols.
• Monitor residents' health status and promptly report changes to RN or physician.
• Provide direct resident care as needed, including wound care, vital signs, and assisting with ADLs.
• Ensure accurate and timely documentation in residents’ charts and medical records.
• Participate in resident assessments and care planning conferences.
• Communicate effectively with residents, families, and the interdisciplinary team.
• Ensure nursing practices are following current federal, state, and local regulations.
• Support infection control and safety procedures according to facility policy.
