Job Title: Licensed Practical Nurse - LTC

Location: Tuck St, Lebanon, PA 17042

Duration: 14-week Contract (possibility of extension)
Shift: 3:00 PM –11:30 PM

Guaranteed Hours: 40

Pay Details:

For Local Candidates (W2): (If your permanent address is within 50 miles from the facility)
Hourly W2 Pay Rate: $38.00
Weekly Gross Pay Rate: $1,520.00

OR

For Local 1099 Candidates:

Pay Rate on 1099: $40.00

Weekly 1099 Pay Rate: $1,600.00

Job Description:

We are seeking a dedicated Licensed Practical Nurse (LPN) to join our team in a long-term care and rehabilitation facility. The LPN will provide compassionate nursing care to residents, assist with daily activities, and support recovery and rehabilitation efforts. This role involves working closely with residents, families, and interdisciplinary teams to ensure high-quality patient care.

Responsibilities:

• Provide direct nursing care, including medication administration and wound care.
• Monitor residents’ health status and report changes to the RN or physician.
• Assist with activities of daily living (ADLs) such as bathing, dressing, and mobility support.
• Implement care plans and ensure compliance with facility policies and regulations.
• Collaborate with healthcare professionals to develop and adjust treatment plans.
• Document patient progress, treatments, and any changes in condition.
• Educate residents and families on health conditions and care routines.
• Maintain a safe and clean environment for residents.
• Support rehabilitation efforts by encouraging independence and mobility.

Requirements:

• BLS (AHA)
• LPN License in the state of Pennsylvania / Compact allowed.
