Duration: 14-Week Contract (With possibility of extension)
Shift: 3:00PM-11:30PM

Guaranteed Hours: 40

OT: After 40 hours

OT Rate: All-inclusive Rate x 1.5

Pay Rate:

For Local Candidates pay rate: $39 to $42/hr. (If your permanent address is within 50-miles from the

facility)

For Travelers/Stipend Candidates (If your permanent address is outside the 50-mile radius from the facility):
Blended Hourly W2 Pay Rate: $52.00
Blended Weekly Gross Pay Rate:  $2,080.00
Minimum Hourly W2 Pay Rate: $21.00
Min. W2 Weekly Gross (Taxable): $840.00
Weekly Stipend (Non-Taxable): $1,240.00

OR

For Traveler 1099 Candidates:
Pay Rate on 1099 to the Candidate: $54.00
Weekly 1099 Traveler Pay Rate:        $2,160.00

Job Description:

We are seeking a dedicated Licensed Practical Nurse (LPN) to join our team in a long-term care and rehabilitation facility. The LPN will provide compassionate nursing care to residents, assist with daily activities, and support recovery and rehabilitation efforts. This role involves working closely with residents, families, and interdisciplinary teams to ensure high-quality patient care.

Licenses:

• BLS Certification.
• LPN License for the State for Vermont/Compact.

Skills required:

• Provide direct nursing care, including medication administration and wound care.
• Monitor residents’ health status and report changes to the RN or physician.
• Assist with activities of daily living (ADLs) such as bathing, dressing, and mobility support.
• Implement care plans and ensure compliance with facility policies and regulations.
• Collaborate with healthcare professionals to develop and adjust treatment plans.
• Document patient progress, treatments, and any changes in condition.
• Educate residents and families on health conditions and care routines.
• Maintain a safe and clean environment for residents.
• Support rehabilitation efforts by encouraging independence and mobility.
