Physical Therapist - Short-Term Rehabilitation Expert

Community Care Center of Hondo in Hondo, TX is seeking a skilled Physical Therapist to join our rehabilitation team. As a PT with Raisso, you will have the opportunity to work with patients who require short-term rehabilitation care.

This position requires a Texas state-licensed Physical Therapist with at least 1 year of experience in long-term care or rehabilitation settings. The ideal candidate will have excellent communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment.

Job Description

As a Physical Therapist at Community Care Center of Hondo, your primary responsibility will be to develop and implement individualized rehabilitation plans for patients with physical functioning disorders and/or wound care needs. You will work closely with other members of the healthcare team to ensure seamless care coordination and effective patient outcomes.

Your duties will include:

• Evaluating patients' physical functioning disorders and wound care needs
• Developing and implementing individualized rehabilitation plans
• Administering various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, and wound care
• Assessing patients' progress and adjusting treatments accordingly
• Providing consultation and counseling to patients, families, caregivers, and other service providers
• Effectively communicating with supervisors and other healthcare team members regarding patient progress, barriers, and treatment plans

Requirements

To be successful in this role, you must possess the following qualifications:

• TX State License - Required
• LTC or Rehab experience - Required
• 1 year of PT experience - Required
