COMMUNITY LIVING COACH
The Community Living Coach (CLC) is responsible for implementation of services to individuals in the Supported Community Living area of the Community Residential Supported Services (CRSS) program. The CLC is also responsible for working to help individuals reach their maximum level of independence within the community. CLCs assist individuals in day-to-day activities and operations of the homes in which they live. CLCs also asist individuals in securing all necessary services such as doctor appointments, transportation, and other services as dictated by the person centered plan.
JOB DUTIES, FUNCTIONS, AND RESPONSIBILITIES :
- Familiarizes self with the principles of Normalization and adopts these principles as working philosophy
- Becomes familiar with Outcome Based Measures as the standard for providing quality services for the individuals served
- Participates in Personal Futures Planning updates
- Provides transportation / assistance to individuals in obtaining services identified in the Personal Futures Plan
- Assists individuals in learning their roles within the larger community (ex.-neighbor, employee, friend, student) and facilitates opportunities for individual to explore these roles
- Provides documentation as directed by the Qualified Developmental Disability Professional (QDDP), Community Living Associate Director or Assistant Associate Director, and / or psychologist
- Provides the support and training necessary for individuals to progress toward a full and normalized life within the community
- Teaches the individuals served to maintain their homes and provides assistance as needed
- Carries out the habilitation goals which the QDDP and the Community Living Associate Director or Assistant Associate Director have developed with emphasis and quality based on outcomes for the individual served
