Attention all Director of Patient Experience! A leading Nursing Home in your area is seeking a compassionate, empathetic and dynamic individual to join our team as a Director of Patient Experience.
As the Director of Patient Experience, you will play a critical role in ensuring the well-being and quality of life for our residents. You will work closely with residents, families, and staff to develop and implement care plans that address their social, emotional and psychological needs for their first couple weeks in the facility.
Responsibilities include :
- Assessing the social and emotional needs of residents and families and developing individualized care plans to address their needs
- Collaborating with interdisciplinary teams to ensure coordination of care and services
- Providing emotional support and counseling to residents and their families
- Assisting residents and families with adjusting to life in a long-term care setting
- Connecting residents and families with community resources and support services
