Key Responsibilities:
- Coordinate and oversee the MDS assessment process, ensuring accurate and timely completion.
- Conduct resident assessments, develop individualized care plans, and ensure appropriate services are provided.
- Collaborate with interdisciplinary teams (nursing, rehabilitation, dietary, etc.) to ensure assessment accuracy and care quality.
- Monitor and analyze resident outcomes, using data to drive continuous improvement in care and services.
- Educate and train staff on MDS processes and regulatory requirements.
Qualifications:
- Active RN license with experience in long-term care.
- Strong knowledge of MDS assessment processes and regulatory compliance.
- Excellent organizational, analytical, and problem-solving skills.
- Strong communication and interpersonal abilities.
