Basic Function

Clinical Services : This 0.6 HEPA position will be shared between the immunization and tuberculosis clinical services teams.Duties will include supporting the programs viaperforming clerical duties including answering phones, responding to voicemails, creating electronic health records charts, assisting clients with registration, scheduling appointments, placing appointment reminder calls, and collecting payment for services. This position will also assist with stocking and general upkeep of clinicals spaces, development and dissemination of educational materials, receiving and sending mail, and if candidate hired is bilingual then assisting with interpretation during appointments.

Administration : This 1.0 HEPA position will be working in the Central Registration area for clinical programs.Duties include responsibility for performing administrative and clerical tasks related to customer service, clinical appointment registration using an electronic health record system, assisting clients with filling out intake forms, using Language Line services when necessary, and confirming their insurance details.

To work with clients in various programs to decrease barriers and improve health outcomes; provide health and well-being education and services; advocate for client needs; help clients navigate health and community systems; build community connections; and to perform related duties as assigned.

Examples of Work Performed

  • Provide health education and health literacy building by carrying out health promotion activities compatible with the language, culture, and customs of clients in order to improve clients' health care by providing culturally appropriate information and education.
  • Assist in addressing the health care needs of clients by completing delegated tasks and performing routine screening tasks in client homes, community settings, or clinics.
  • Assist in assessing community health needs by meeting with community groups to determine the community's needs for education and service, and provide education.
  • Assist in the development of health promotional activities by helping to plan, develop, and implement : programs, displays, printed materials, audio-visual aides, and oral presentations that will improve clients' understanding of and access to appropriate services.
  • Provide direct service as delegated by health and human services staff; provide education and support for families including role modeling, guidance, and problem solving.
  • Teach good health practices by leading discussions on special topics with individuals, families, or groups in ways that are appropriate to the language, culture, and customs of clients.
  • Perform duties related to client / patient screening procedures, records, appointments, processing forms and reports, and the collection and dissemination of medical information; assist in the maintenance and preparation of exam areas, medical equipment, client / patient records, and medical forms; and assist patient intake staff and other ancillary personnel.
  • Transport clients to medical and social service appointments in county owned or personal vehicles or assist in using public transportation.
  • Provide crisis intervention, direct service, and advocacy to meet immediate needs and longer term goal support.
  • Provide Direct Observational Therapy (DOT) when providing medications to patients in their homes; work with case management staff to report patient concerns; and document daily DOT interactions in the patient's chart.
  • Compile, document, and maintain information related to the operations of specific programs.
  • Meet individually and collectively with program participants when assigned to obtain necessary factual information, answer questions, or discuss problems. Notify clients about county programs and benefits , and assist clients with obtaining services.
  • Plan and schedule equipment and facilities for program activities; in some cases set up and take down equipment for public presentations and training classes.
  • Investigate and arrange for available community resources which can be included in the program to avoid duplication of services.
  • Design forms, maintain accurate and thorough records of program activities, and prepare reports of activities in progress as required.
  • Assist in conducting needs assessments, gather data, develop proposal strategies, and present proposals.
  • Assist in evaluating effectiveness of program services and materials by suggesting improvements or additions to existing programs.

Minimum Qualifications

Education : High school graduation or equivalent.

Experience : One year of experience working in a health care capacity in a health care or health education setting or equivalent.

Substitution : Completion of certification requirements for Community Health Worker may substitute for the one year of experience requirement.

Certifications / Licensure : May require a driver's license or equivalent ability to travel based on program needs. Community Health Worker Certification may be required depending on assignment.

Other : If assigned to Tuberculosis clinic must be able to be fit-tested and able to wear a mask.

The examination process will consist of the following section with each section weighted as indicated :

  • Training and Experience Rating = 100%
  • The examination for Community Health Worker / Health Education Program Assistant will consist of a training and experience rating, comprised of the questions in the attached supplemental questionnaire. Not all applicants who meet the minimum qualifications will pass the training and experience rating. The rating on this supplemental questionnaire will depend your answers to the questions - do not mark "see resume" or "see work history" as a response. Please answer all your questions clearly and completely. Failure to respond could affect your score and final rank on this examination.

    Eligible List : The names of all applicants who pass the examination / screening process and file a properly completed application shall be placed on the eligible list for employment as a Community Health Worker / Health Education Program Assistant. This list will be certified to the appointing authority which may use this list to fill a vacancy. Candidates will remain on the list for two months or until hired, whichever occurs first. A notice will be sent to applicants at the time the eligible list is posted, informing them that the list has been posted and their rank on the list.

    Veteran's Preference : This is a classified position requiring an open, competitive selection process . Veteran's Preference points will be applied after a candidate passes the examination process. Applicants who are eligible for veteran's preference should update their veteran's DD214, and other supporting documents, and submit them as an attachment at the time of application.
