Responsibilities of MDS / Care Plan Coordinator :
Actively participate in the development of a comprehensive, individualized plan of care for each resident.Ensure resident plan of care is completed, updated, and reviewed in accordance with the RAI guidelines and changes in resident care / needs.Assume responsibility for implementation of Clinical and Prospective Payment System Minimum Data Set completions scheduleDirect, organize and schedule required resident assessments and related documentation for Medicare, Medicaid, managed care and Health Maintenance Organization (HMO) reimbursementCoordinate and oversee all functions of the MDS assessment process to assure compliance with federal and state requirementsParticipate in pre-admission process to determine potential residents' level of careParticipate in resident level of care determination upon admissionParticipate in tracking changes on residents' level of care to assure timely scheduling of MDA assessmentsReview all MDS assessments and related forms to certify timely completion by signing the MDS forms on the appropriate sectionsReview reports of timely completion of residents' MDS assessments and care plans and take follow up action as necessaryAnalyze Quality indicator reports for potential resident concerns and problems and review residents' care plans to be sure this information is includedWill maintain a current knowledge base regarding state and federal regulations relating to the RAI process, care planning and the prospective payment system (PPS).Qualifications of MDS Coordinator :
RN / LPN LicenseMDS ExperienceABOUT US :
