Position Title: Social Worker
The purpose of this position is to provide services that meet the social and/or emotional needs affecting the resident's ability to achieve their highest level of function; participate in the development of resident comprehensive care plans; develop policies and procedures to provide social services to residents in compliance with federal, state and local regulations. Any facility with more than 120 beds must employ a qualified social worker on a full-time basis.
Qualifications
- Licensure or certification by the State as a Social Worker
- Minimum of bachelor's degree in Social Work or a human services field including, but not limited to, sociology, gerontology, special education, rehabilitation counseling and psychology and
- 1 year of supervised social work experience in a health care setting, working directly with individuals
Delegation of authority
Authority is delegated to the individual in this position to:
Develop policies and procedures to provide social services to residents in compliance with federal, state and local regulations.
Interview, assess and develop individualized plans to meet resident needs.
Assess the facility's social service program and make recommendations to the Administrator.
This position reports to: Administrator
1. Develop and implement social service policies and procedures to meet resident needs in compliance with federal, state and local regulations.
2. Develop, coordinate and participate in family and resident activities designed to promote social interaction, reality orientation and intellectual stimulation.
3. Develop one-to-one professional relationships with residents and families as needed for counseling.
4. Counsel residents and families in dealing with feelings about death or dying as well as other emotional, mental, environmental or physical limitations.
5. Assess and complete the sections of the (MDS) Minimum Data Set assigned completely, timely and accurately.
6. Document the social service component of the comprehensive Care Plan for each resident in a timely manner.
7. Document psychosocial needs, interaction and follow-up action(s) with resident and/or family according to facility policies and procedures.
8. Assess, plan and document resident discharge needs in accordance with facility discharge planning policy and regulatory requirements.
9. Act as a liaison with social, health and community agencies.
10. Refer residents to social, health and community agencies and complete accurate documentation in resident records concerning the results of such referrals.
11. Coordinate family and community resources as needed to solve financial needs and to promote emotional security.
12. Take part in facility in-service programs as required.
13. Organize community events related to resident needs and interests.
14. Explain resident's rights to residents and resident's representatives.
15. Participate in admission of residents as assigned.
16. Plan, schedule and implement room visits for residents unable to leave their rooms.
17. Organize and participate in the theft loss program.
18. Coordinate and cooperate with the activity department to plan and conduct events to meet socialization needs of the residents.
19. Observe all facility safety policies and procedures.
20. Accept assigned duties in a cooperative manner.
21. Perform other related duties as directed by his/her supervisor.
22. Perform all duties assigned in an effective, timely and professional manner.
23. Follow infection prevention and control procedures at all times.
24. Follow Resident Rights policies at all times.
25. Be responsible for the safety of residents under his/her supervision.
26. Participate in implementation of programs to improve resident care related to Quality Measures.
27. Assume Quality Assurance duties as assigned.
28. Observe all facility policies and procedures.
29. Assume accountability for all data contained in the employee handbook.
30. Report for work as scheduled and consistently demonstrate dependability and punctuality.
31. Report for work in neat, clean attire and consistently present an appropriate professional appearance.
32. Consistently work cooperatively with residents, residents' representatives, facility staff, physicians, consultants and ancillary service providers.
33. Agree not to disclose assigned user ID and password for accessing resident/facility information. Promptly report suspected or known violations of such disclosure to the Administrator.
34. Agree not to disclose resident's protected health information and promptly report suspected or known violations of such disclosure to the Administrator.
35. Report any known or suspected unauthorized attempt to access facility's information system.
36. Report suspected or known incidents or fraud relative to false billings, cost reports, kickbacks, etc.
37. Ensure that department computer workstations left unattended are properly logged off or the password protected screen saver activates within established facility policy guidelines.
38. Report all accidents/incidents to your supervisor no matter how minor they may be. Report such occurrences on the shift in which they occur.
39. Report suspected resident abuse immediately to the Administrator.
