Maximum of 2 Days Call off (Unapproved RTO) during contract.\n\nTitle : Care Coordinator (case manager) nurse positions within our Primary Care clinic : \n\nRequirement : Three (3) year Case Management Experience.\n\nShift : Weekdays Monday through Fridays from 8a to 5p, with One (1) hour lunch, no weekends or holidays. Collaborate with support nursing staff and with providers for patient care.
