What are the responsibilities and job description for the LPN position at Rapha Centre?
JOB SUMMARY
The LPN will be responsible for clinical and administrative duties under the direction of a physician, nurse practitioner, registered nurse or other qualified clinical person. Clinical duties include taking and recording vital signs and medical histories, preparing patients for examination, drawing blood, and administering medications as directed. Administrative duties include maintaining medical records and coding for insurance purposes. The LPN is an integral and synergistic member of the overall recovery team while adhering to the policies and procedures of Rapha.
REPORTING RELATIONSHIPS
The LPN will report to the Nursing and Lab Coordinator for routine supervision, daily assignments and staffing.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Taking and recording the patient’s medical history
- Taking vital signs, such as blood pressure, temperature, and weight
- Giving medication as prescribed by a provider
- Giving injections of medications, Immunizations, etc.
- Entering information into the EHR/computer system
OTHER DUTIES AND RESPONSIBILITIES
The LPN will advocate for the patient while working with the medical or mental health providers. H/she must have the ability to handle a diverse range of situations and responsibilities.

- Assist MAs in completing Urine Drug Screens(UDS) and UDS reports.
- Complete Prior Authorizations (PAs) for medications and follow up as required/appropriate.
REQUIRED EXPERIENCE, LICENSES AND/OR CERTIFICATIONS
- Must be a Licensed Practical Nurse in the State of Tennessee with a minimum of one year of related experience.
- Must be certified in and maintain certification in CPR.
- Must have a valid Drivers License (TN).
- Phlebotomy certification preferred, experience required.
CONDITIONS OF EMPLOYMENT
Position is hourly and pay will be commensurate with experience and education.
Must submit to random drug testing and prefer knowledge of the 12-step program.
Working hours 7:00am – 5:30pm, M-F.
PHYSICAL DEMANDS/WORK ENVIRONMENT
- Standing for long periods regularly
- Occasionally required to sit, climb, balance, stoop, kneel, crouch, or crawl
- Must regularly lift and/or move up to 10 pounds
- Potential exposure to moving mechanical parts, fumes, or airborne particles.
