Overview
We are seeking a dedicated Registered Nurse Case Manager to join our healthcare team. The ideal candidate will have a strong background in hospital medicine, infectious disease care, and geriatrics. This position offers the opportunity to work in a Level II Trauma Center, providing care to patients across various settings such as hospitals, clinics, and outpatient facilities.
Responsibilities
- Conduct diagnostic evaluations and assessments for patients
- Develop and implement individualized care plans for patients
- Coordinate with healthcare providers to ensure comprehensive patient care
- Monitor patient progress and adjust treatment plans as needed
- Provide education and support to patients and their families
- Collaborate with interdisciplinary teams to optimize patient outcomes
Experience
- Previous experience working as a registered nurse in a hospital or clinical setting
- Strong knowledge of physiology and medical terminology
- Experience in geriatric care is preferred
- Familiarity with case management principles and practices
- Ability to work effectively in a fast-paced environment
- Excellent communication and interpersonal skills
Job Type: Contract
