OVERVIEW

Implements individualized social and living skills development program and provides direct care including supportive level counseling and skill training for individuals who have serious and persistent mental illness and live in a supervised residential facility on a 24 / 7 basis. Work is performed under regular supervision and is guided by well-established and specific agency policies and procedures, quality assurance, public and private payor guidelines, professional practices and including ethical standards related to client interactions, state and federal licensure and accreditation regulations and guidelines.

DUTIES AND RESPONSIBILITIES

  • Instructs residents in basic independent living skills including : mobility; hygiene and self-care; home maintenance and safety; nutrition and home economics; social behavior and communication and functional academics; personal banking and money management, etc.
  • Leads residents in social and leisure activities.
  • Attends and assists residents with physician appointments.
  • Supervises the self-administration, or administers medication, and prepares and maintains related records, monitors resident medication management and coordinates with staff, prescribing physicians and pharmacies.
  • Transports residents.
  • Records residents' progress notes and communicates resident's behavior to relieving staff members.
  • Provides supportive level counseling and crisis intervention.
  • Provides overnight supervision and support to residents.
  • Assists in the upkeep of the program facility, grounds and vehicles.
  • Conducts emergency drills according to policy, and completes appropriate records.
  • Monitor individual service plans in collaboration with resident, his / her family, and other service providers, as appropriate, under supervision of the Supervised Apartment Manager.
  • Provides timely and accurate documentation of Individual Service Plans, telephone contacts, funding requirements, DBHDS requirements, and other information to meet program, agency licensing and Medicaid billing requirements.
  • Assists in the relocation of residents and personal belongings.
  • Attends required and related trainings.
  • Completes incident reports and other required documentation as incidents occur.

KNOWLEDGE, SKILLS, AND ABILITIES

Considerable knowledge of the practices and methods related to the instruction of the long-term mentally ill population including behavior management, independent living skills training and supportive counseling; considerable knowledge of human rights of the individual; considerable knowledge of common housekeeping practices; some knowledge of the operation of electronic medical records and associated documentation; ability to physically assist residents; ability to prepare accurate resident records; ability to establish rapport with SMI individuals and their families; ability to establish and maintain an effective working relationship with other staff; ability to communicate effectively orally and in writing; ability to function calmly in stressful situa­tions; ability to observe, assess and record individual behaviors and functioning; ability to operate a vehicle; ability to effectively respond to crisis situations both behavioral and medical.

TYPICAL EDUCATION, TRAINING AND WORK EXPERIENCE

High School Diploma, coursework in the Behavioral Sciences, and at least 1 year experience providing direct service to persons having a diagnosis of mental illness.

LICENSURE AND CERTIFICATIONS

Valid Virginia driver's license. Certification in First Aid / CPR. Training in Therapeutic Options (THEROPS).
