What are the responsibilities and job description for the CME Care Coordinator position at Ravenwood Health?
- Acquires technical knowledge about youth served and demonstrates the ability to apply that knowledge in understanding and assisting with client difficulties. Remains knowledgeable and current on research/information as it pertains to the population served.
- Regularly engage with youth and families on a community-based level in a culturally competent manner.
- Assess level of care using the CANS assessment or link youth and families to a CANS assessor in their area.
- Connect youth and families/caregivers to resources and natural supports and work collaboratively with wraparound teams.
- Organize wraparound team meetings and maintain documentation of these meetings.
- Link youth and families to specialized clinical resources to meet their treatment needs.
- Provides assistance and support in crisis situations; responds effectively and is able to mobilize access to resources in crisis situations.
- Works closely with youth and families to develop care plans that incorporate their voice and choice, and regularly monitors and adjusts care plans as necessary.
- Collects outcome information by administering and collecting necessary forms to client and family.
- Assists in transportation of youth and their families.
- Accepts 24 hour on-call responsibilities as assigned.
- Participation in regular training regarding health equity/health disparities and cultural competence
- Have a minimum of three years’ experience in children’s behavioral health, child welfare, developmental disabilities, juvenile justice, or a related public sector human services or behavioral health care field, providing community-based services to children and youth and their family or caregivers.
- Background and/or experience in one or more of the following: family systems, community systems and resources, case management, child and family counseling or therapy, child protection, or child development preferred.
- Cultural knowledge of key populations that will be served preferred.
