Licensed Practical Nurse

Job Description

Rayrac Healthcare Services is seeking a dedicated Licensed Practical Nurse to join our healthcare team in the Douglasville area and surrounding areas. The ideal candidate will have experience in home health setting with proficiency medical documentation, Trach, catheter and G-tubes.

Education and Experience: Must have an active practical nursing licensure (Georgia); Experience in home health or group home care setting is preferred.

Essential Duties And Responsibilities

• Provide direct patient care in home health and group home settings
• Administer medications and treatments as prescribed by physicians
• Assist with G-tube feeding , catheter care and medication management
• Perform medical documentation accurately and efficiently
• Collaborate with the healthcare team to deliver high-quality patient care
• Complete evaluations, assessments, and treatments in accordance with policy and the individualized care plan
• Complete all documentation and records in a timely and accurate manner
• Observe basic physical, functional, behavioral and / or psychosocial conditions, and analyze medical history;
• Knowledge of proper Medication Administration techniques and basic pharmacology.
• Ability to achieve and maintain certifications necessary for the job (including, but not limited to, First Aid, CPR, MANDT, Medication Administration).
• Skilled in performing nursing assessments and emergency medical procedures.
• Attention to detail in medical documentation
• Assess and document the health status needs of individuals supported by Rayrac Healthcare Services;
• Assists with the facilitation and implementation of health care objectives of the Person-Centered Plan, collaborates with members of the Interdisciplinary Team (IDT) and other health care professionals in the community.
• Complies with all applicable regulatory agency regulations and procedures as well as the policies and procedures of the agency.

• BLS Certification (Preferred)
• LPN (Preferred)
  
  

Other: 21 years or older, Maintains valid driver’s license and insurable under their auto insurance. Ability to pass criminal/registry background checks. Clear Tuberculosis screening
