Job Opportunity: CAP-C & CAP-DA Case Manager for Iredell, Rowan, Cabarrus and surrounding counties
We are currently seeking a CAP-C (Community Alternative Program for Children) and CAP-DA (Community Alternative Program for Disabled Adults) Case Manager. This is a hybrid position that requires travel to client locations as needed. Responsibilities:
- Case Management: Manage and coordinate services for individuals receiving CAP-C and CAP-DA services.
- Assessment and Care Plans: Conduct assessments and develop individualized care plans for patients.
- Collaboration: Work with families, medical professionals, and service providers to ensure the best possible outcomes for patients.
- Timely Completion of Tasks: Ensure all documentation, reporting, and tasks are completed on time.
- Travel Requirements: Travel to client homes or facilities as necessary for assessments, meetings, and care coordination.
- Compassionate Support: Provide compassionate care and support to special needs patients and their families.
Qualifications:
- Experience: Previous experience in case management, specifically in CAP-C and/or CAP-DA services, preferred.
- Bachelor’s degree in social work from an accredited school of social work, and one year of directly related experience of community experience (preferably case management) in the health or medical field directly related to homecare, long-term care, or personal care, and the completion of a NC Medicaid-certified training program within 90 calendar-days of employment;
- Bachelor’s degree in a human services or equivalent field from an accredited college or university with two or more years of community experience (preferably case management) in the health or medical field directly related to homecare, long-term care, or personal care and the completion of a NC Medicaid-certified training program within 90 calendar-days of employment;
- Bachelor’s degree in a non-human services field from an accredited college or university with two or more years of community experience (preferably case management) in the health or medical field directly related to homecare, long-term care, or personal care and the completion of a NC Medicaid-certified training program within 90 calendar-days of employment; ord.
- Registered nurse who holds a current North Carolina license, two-year or four-year degree, one year case management in homecare, long-term care, personal care or related work experience and the completion of a NC Medicaid-certified training program within 90 calendar-days of employment
- Communication Skills: Strong verbal and written communication skills.
- Self-Motivated: Must be able to complete tasks independently and meet deadlines.
- Compassion: Demonstrated passion for working with medically fragile beneficiaries and their families.
- Travel: Ability to travel to various client locations as required.
