JOB SUMMARY:
Assists the Director of Nursing in planning, organizing, developing and directing the day
to day functions of the nursing department in accordance with current facility local, state
and federal regulations.
ESSENTIAL DUTIES & RESPONSIBILITIES:
• Supervises and is responsible for other nurse leaders, managers, supervisors, as
well as licensed professionals and para-professionals within the nursing
department. Ensure they follow established departmental policies and
procedures.
• Adheres to master staffing plan.
• Works as needed, schedule to be flexible, as approved by the Director of Nursing
and the Administrator.
• Assumes all responsibilities of the Director of Nursing in their absence.
Familiarity of these functions is achieved through close association and constant
communications regarding such responsibilities on a day to day basis with the
Director of Nursing.
• Directs and supervises the implementation of resident care policies.
• Plans, organizes, implements, evaluates, coordinates and directs an effective
infection control program in accordance with all policies and regulations, as
applicable per facility roles.
• Serves as Workers Compensation Coordinator, as applicable, per facility roles.
• Coordinates pharmacy services.
• Participates in MDS/CMI management.
• Issues appropriate preliminary disciplinary action to nursing staff as needed.
• Mediates family, visitor and staff concerns.
• Assumes role of shift supervisor in the absence of such.
• Maintains resident rights.
• Maintains HIPAA, privacy, security and confidentiality standards.
• Follows all established policies and procedures.
• Maintains Corporate Compliance standards and annual training.
• Follows safety procedures by keeping areas free of hazardous conditions.
• Is economical and careful with supplies and equipment.
• Attends all required in services/trainings.
• Comes to work as scheduled and consistently demonstrates dependability and
punctuality.

Regulatory Compliance:
• Reports all reportable diseases and rates of infection over threshold to the NYS
Department of Health as per policy/regulation.
• Participates in NYS Department of Health and Federal Surveys.
Care and Treatment of Residents:
• Reports to the Director of Nursing to convey resident status and other conditions
related to the nursing department.
• Reviews the 24-hour report. Obtains a report of resident’s condition from rounds
on the nursing units, and reports pertinent information to the Director of Nursing.
• Review all laboratory reports, making notation of all abnormal results and
assuring that the attending physician (provider) is to be notified.
• Initiates and compiles infection surveillance forms, as indicated by abnormal
culture and sensitivity reports to ensure treatment and resolution of said infection.
• Reviews all resident incident reports, giving recommendations for prevention and
other actions as deemed necessary.
• Monitors changes in resident’s condition and addresses any other circumstances
that warrant attention.
• Attends resident comprehensive care planning meeting as requested.
• Attends meetings as required or invited, ie: morning meeting, skin, A&I, antibiotic
stewardship, etc.
• Ensures that all infection control precautions and aseptic techniques are
practiced appropriately, taking corrective action when needed.
• Makes rounds to nursing units for purpose of case findings, review of sanitation
procedures and supervises isolation/precaution practices.
• Performs investigations of nosocomial infections. Traces possible sources of
infection in the facility. Institutes appropriate actions to confine infections, as
applicable per facility roles.
• Assists in developing and implementing guidelines for decontamination and
sterilization in the facility.
• Reviews and analyzes infectious disease laboratory reports and consults with
Director of Nursing to ensure appropriate techniques are being utilized.
• Personally, greets new admissions on the next business day following admission.
Quality Assurance Performance Improvement:
• Attends QAPI meetings as an active member.
• Prepares assigned reports for QAPI meetings.
• Contributes to facility Quality Assurance Performance Improvement initiatives.
• Provides leadership and commitment to improving the use of safe and
appropriate antibiotics in the facility (Antibiotic Stewardship).
• Assures the quality assessment and the utilization of acceptable surveillance
definitions and guidelines for infection monitoring and identifies situations where
antibiotics should not be used.
• Assures adherence to tracking and monitoring antibiotic prescribing.
• Provides educational resources and materials to clinical providers, nursing staff,
residents and families to learn about antibiotic resistance and opportunities for
improving antibiotic use.
• Maintains a written record of all nosocomial infections.
• Prepares monthly summaries of resident/personnel infections, corrective action
taken, and results of corrective action.
REQUIRED SKILLS & ABILITIES:
• Proficiency in various computer programs, applications, electronic
communications, electronic medical records.
• Must possess strong knowledge regarding state, federal and local regulations as
they pertain to long term care.
• Must be able to communicate in English, both verbally and in writing, and
possess sufficient communication skills to perform the tasks required.

QUALIFICATIONS:
EDUCATION:
Minimum: Associates Degree in Nursing
Preferred: Bachelor’s degree in Nursing
EXPERIENCE:
Minimum: 2 years’ experience in supervisory nursing position in a Long-Term Care
setting
Preferred: 3 to 5 years’ experience in a supervisor nursing position in a Long-Term
Care setting
LICENSURE/CERTIFICATION: Valid NYS Registered Nurse or LPN Licensure
