Job Summary:
As a Licensed Practical Nurse (LPN), you’ll provide compassionate, high-quality care tailored to our adult and geriatric residents' specific needs. You’ll ensure every resident receives individualized care in alignment with their personalized care plan.
Key Responsibilities:
- Deliver exceptional care to residents, addressing physical, emotional, and mental needs.
- Administer medications, treatments, and services with precision and documentation accuracy.
- Participate in interdisciplinary care planning, facility meetings, and quality improvement initiatives.
- Supervise nursing and support staff, fostering a positive and efficient working environment.
- Respond to medical changes, family concerns, and ensure communication with care providers.
- Ensure HIPAA compliance and uphold resident rights and safety standards.
Preferred Skills & Qualifications:
- Expertise in managing care for patients with therapeutic, neuro, dementia, dialysis, wound care, or end-of-life needs.
- Strong understanding of infection control, behavioral interventions, and emergency response.
- Exceptional communication and teamwork skills, with a proactive and optimistic attitude.
