SHIFT: Full-Time Days , 8am - 4:30pm, (40 HOURS A WEEK). WEEKENDS AND EVENINGS NEEDED AS OPERTIONALLY NECESSARY
JOB DESCRIPTION:
The Unit Manager oversees our subacute and long-term care units, assesses residents, and supports/directs staff as needed. Unit managers are responsible for managing the nursing unit. They primarily supervise the daily care of patients and ensure quality patient care.
RESPONSIBILITIES:
- Demonstrates knowledge of age specific developmental factors specific to the adult and geriatric population in the planning and delivery of care
- Demonstrates knowledge, skills and techniques necessary to care for residents with the following needs: therapeutic, neuro and dementia, dialysis, IV therapy, infectious disease, wound care, end of life care, if applicable
- Assures completion of a comprehensive nursing evaluation of a resident’s physical, emotional and mental status upon admission/re-admission and when indicated, as well as, appropriate follow-up as indicated by the evaluation.
- Acting as the main point of contact between residents and their families
- Addressing and preventing concerns from family members
- Assists in developing, implementing, and making adjustments to patient care plans as needed
- Supervises the staff nurses, nursing rehab assistants, certified nursing assistants, unit clerks (if applicable), and completes annual and probationary evaluations of same
- Ensures their unit meets federal and state regulatory standards, as well as company policies and procedures
- Collaborates with the Director of Nursing, Nurse Practitioners, and Physicians to plan and maintain resident care
- Completes admission and discharge assessments
- Oversees and audits changes made to care plans
- Delivers hands-on care and medicine distribution as needed
REQUIREMENTS:
- Valid NYS Registered Nursing License or LPN License
- Minimum: 1-year experience as an RN or LPN in a long-term care setting
- Preferred: 3-5 years as an RN or LPN in a long-term care setting with supervisory experience
- Education: Minimum: Associates Degree Preferred: Bachelor Degree

- Excellent Communication Skills, coordinating activities in conjunction with other departments to ensure residents feel safe and empowered. Interacts positively with family members, updating them on how resident is doing.
- Displays excellent Customer Service by assisting and cooperating with other personnel and interdisciplinary teams exhibiting helpfulness and support to all
team members. Provides direction to visitors, staff, volunteers and external populations - Problem solver identifying actual/potential problems and creates an action plan for resolution and then measuring outcomes to determine course of action.
- Able to perform tasks while upholding high standards of quality, cleanliness, and kindness
- Physical endurance for frequent activity; ability to lift a minimum of 20 pounds
