Intensive Care Manager (RN) – Behavioral Health
Requires 3-5 visits to members per week
Travel within Hartford and Northeastern CT or Fairfield County
When not making a visit, position is remote.
Salary: $87,000 (DOE) and Comprehensive Benefits
A not-for-profit managed care organization dedicated to improving medical services and health outcomes for Connecticut’s Medicaid-eligible residents.

Travel within Hartford and Northeastern CT or Fairfield County
Responsibilities:
- Collaborates with the Health Plan’s clinical team, provider network, DSS and the community to create a process of assessment, planning and facilitation to meet the member’s heath care needs across the continuum of care.
- Assists the member by functioning as an advocate for options and services by communicating effectively with other members of the healthcare team and by utilizing available resources to promote quality cost effective outcomes and member autonomy.
- Develops, monitors and evaluates the effectiveness of the care plan and modifies as necessary.
- Provides education and coordination of outpatient care to Health Plan members selected for Case Management.
- Identifies and refers quality of care, coordination of benefits and third party liability issues as appropriate.
- Orients new staff as requested.
- Engages with members through telephonic, videoconferencing and face-to-face outreach.
- Conducts assessments to identify the member’s needs and develops a specific care plan to address objectives, barriers and behaviorally specific goals identified during the assessment.
- Maintains knowledge of covered benefits for CT Medicaid, HUSKY Health programs.
- Performs other duties as assigned.
Requirements:
- Valid and unrestricted CT State RN License
- 2 years direct clinical patient care. Behavioral health experience and knowledge of case management are beneficial.
- Managed care and case management experience preferred.
- CCM or other URAC approved certification preferred or within 4 years of hire.
- Requires excellent verbal and written communication skills.
- Good time management and organizational skills along with the ability to manage continually changing priorities. Strong computer and keyboard skills.
- Ability to exercise independent and sound judgment in decision making.
- Must hold a valid and unrestricted motor vehicle operator’s license and have reliable transportation.
- Proof of vaccination against Covid is required.
