Job Title : Social Worker – PACE Program
Location : Oceanport, NJ
Salary : $68,000 - $75,000 per year
Hours : Monday - Friday
8 : 00am - 5 : 00pm

About the Social Worker Role :
We are seeking a compassionate and dedicated Social Worker to join the interdisciplinary team at a leading PACE (Program of All-Inclusive Care for the Elderly) program in Oceanport, NJ. In this Social Worker role, you will provide essential social work services to participants and their families, ensuring they receive the support and resources needed to remain in the community while maintaining their health and quality of life.
Key Social Worker Responsibilities :
- Perform initial and periodic assessments (every six months or as needed) of participants, contributing to individualized care plans.
- Deliver case management services, including counseling, education, and advocacy for participants and their caregivers.
- Maintain accurate and timely documentation in compliance with industry standards.
- Develop and lead group counseling sessions and activities to support participant well-being.
- Coordinate with community resources and agencies to meet participants’ needs effectively.
- Provide psychosocial support, address mental health needs, and manage crisis situations as they arise.
- Act as a liaison between participants, families, caregivers, and the interdisciplinary team to ensure clear communication and collaboration.
- Participate in home visits to assess participant environments and provide necessary support.
- Engage in program development, quality improvement initiatives, and policy refinement.
