The UAS Assessment Nurse (RN) plays a crucial role in evaluating the clinical, psychological, and social needs of new and existing members. Utilizing the Uniform Assessment System for New York (UAS-NY), you will conduct in-home and nursing home assessments, ensuring compliance with New York State Department of Health (DOH) regulations.
Key Responsibilities
- Conduct initial and reassessment visits, determining eligibility and care needs
- Explain program benefits, rules, and services to members and caregivers
- Perform functional, psychosocial, and environmental safety assessments
- Develop and communicate individualized person-centered service plans
- Submit accurate and timely documentation with an error rate below 5%
- Work within DOH-mandated timeframes (minimum two assessments per day)
- Identify and communicate urgent care needs to the care management team and PCP
- Associate's Degree in Nursing (Bachelor’s preferred)
- Minimum one year of experience in Community Health, Managed Care, or a related field
- UAS-NY certified
- Proficiency in Microsoft Word & Excel
