Documentation Needed to Apply
To be considered, you must submit the following:
- Resume with health care experience.
- CNA or PCA certification from an accredited program.
- Valid driver’s license if providing client transportation.
- TB Test with negative results or willingness to be tested.
- CPR and First Aid certifications.
- Minimum one year of experience in caregiving (home care, hospital, or nursing home settings).
Essential Job Responsibilities:
Deliver Compassionate Care: Provide medical and personal care to clients in accordance with the individualized Plan of Care prepared by the Nurse Supervisor, ensuring the highest standard of safety, well-being, and satisfaction for clients and their families.
Monitor and Report Client Status: Promptly communicate any changes in client medical conditions, emergencies, or behavioral concerns to the Nurse Supervisor to ensure proper case management and appropriate interventions.
Maintain Open Communication: Provide timely updates regarding any changes in caregiver or client schedules, including tardiness, vacation requests, time-off needs, status changes, or hour adjustments, to the Staffing Coordinator to guarantee seamless scheduling and coverage.
Facilitate Transportation: Drive or escort clients to and from appointments or activities, using the client’s vehicle as needed, to ensure safe and timely transportation while maintaining a professional and caring demeanor.
Adhere to Schedule Protocols: Confirm monthly schedules with the Staffing Coordinator to ensure consistent coverage and avoid gaps in care services.
Promote Client Comfort: Assist clients with daily tasks, including personal hygiene, ambulation, meal preparation, shopping, light housekeeping, and medication reminders, fostering a clean and safe living environment.
Respond to Emergencies: Act quickly and responsibly in emergency situations, providing immediate care and notifying the appropriate medical personnel or agency contacts as needed.
Other Duties as Assigned: Be prepared to take on additional responsibilities as requested to support the client, agency, and team.
