The Day Habilitation Service Coordinator is responsible for ensuring compliance with CARF and waiver regulations. This position is responsible for the health and safety of the members as well as evaluating the program for its ability to meet the individual needs of each member. The direct supervision of all program staff, scheduling of staff, and on-going staff training for the Day Habilitation are also handled by this position. The Service Coordinator also works directly with the members as needed.
ESSENTIAL JOB FUNCTIONS:
- Train, supervise, discipline and evaluate Direct Care Staff and Facilitators, Direct Care Staff Assistants, interns, and volunteers in conjunction with Program Director.
- Coordinate ongoing staff orientation and education programs specific to the Day Program in conjunction with the Member Health Care Coordinator and the Program Director.
- Complete and oversee the scheduling of the Day program staff; Review timesheets for accuracy.
- Attend weekly professional staff meetings and conduct weekly staff meetings as well as keep record of the meetings.
- Implement, coordinate, and maintain health and safety for the Day Program, including conducting monthly drills and safety meetings.
- Ensure policies and procedures pertaining to member services and staff policies and procedures are followed with an emphasis on adherence to Medicaid, Waiver, County, and CARF accreditation requirements.
- Tour, acquaint, and orient new members to the Day Program.
- Plan programs to meet individual needs of members, attend Individual Service Plan annual meetings and review meetings as needed.
- Complete the Individual Service Plan and review quarterly progress reports.
- Oversee the schedule of social, leisure, recreation, learning activities, and group activities in the community and tracking the % of time in the community and at the Day Habilitation.
- Oversee the maintenance member records, including attendance records, financial records, progress notes, quarterly progress reports, and goal checklists with the assistance of the Day Habilitation Facilitator.
- May assist with daily activities including toileting, feeding, hygiene needs, and recreational activities. Transport members as needed.
- Treat co-staff and supervisors with dignity and respect by listening to what others have to say and receiving constructive criticism.
- Drives team-related work and respects the ideas and opinions of team members. Presents themselves in a professional manner.
- Maintain contact with parents, guardians, and external stakeholders.
- Respond to concerns within 48 hours, with a response time dependent on the nature of the concern.
- Attend other meetings requested by the Program Director or Executive Director.
- Be on time and work scheduled days.
- Complete other duties as assigned.
JOB REQUIREMENTS:
EDUCATION/ EXPERIENCE:
Bachelor’s Degree from an accredited college or university in the social or behavioral sciences and one year of post-degree experience in the delivery, planning, coordination, or administration of human services. Degree may be substituted with four years of work-related experience.
SKILLS:
Must be able to communicate effectively with members and staff. Patience in working with members is necessary. Needs to have a caring attitude and be supportive and encouraging to members. A good driving record is necessary.
SUPERVISORY RESPONSIBILITIES:
Direct supervision over Day Habilitation Facilitator, Direct Care Staff, Direct Care Staff Assistants, volunteers, and interns.
EQUIPMENT TO BE USED:
Must be able to operate computer and other office machines such as fax, calculator, telephone, etc. Training for specialized adaptive equipment for individual members will be provided.
TYPICAL PHYSICAL DEMANDS:
It is preferred that the employee is able to lift at least 50 lbs. Must be willing and able to assist with toileting and other activities of daily living as required.
WORKING CONDITIONS:
Participates in weekend shift rotation and Service Coordinator on call rotation. Occasionally called upon to work overtime or odd schedules. Typically works in both an office setting and direct care at the Day Habilitation, although may be called upon to work direct care at a residential location if necessary.
