Reports to: The Program Director (PD)

Supervision of: All staff assigned to the program under the direction and supervision of the PD

Hours: 40 hours per week to include some direct care, Schedule to be determined and will include 1 evening and 1 weekend or 2 evening shifts

Values: APDs are responsible to exhibit and display the agency’s mission and basic philosophy of consumer validation and empowerment through respectful interaction. They must ensure that program staff adheres to agency policies and procedures and DDS regulations

Responsibilities:

All responsibilities are under the direction, supervision and in conjunction with the PD

  • APD’s are responsible for the day to day functioning of the program to which they have been assigned under the direction of the PD
  • Ensure quality care of all individuals
  • Assist PD in securing adequate program coverage
  • Runs the staff meetings twice monthly in the absence of the PD
  • The APD is the Human Rights Officer for the program. Must attend training to become certified and represent the program at quarterly HRC meetings. Must also be available to address potential human rights issues with consumers
  • Maintain current mandatory training certifications
  • Under the supervision of the PD responsible to oversee all documentation especially the Confidential Files. (communication log, staff timecards, request for time off, client data sheets, daily and monthly progress notes, medication sheets)
  • Supervise case management for monthly progress notes
  • With approval of PD, arrange and attend appointments with all individuals.
  • In conjunction with the PD, the APD will be responsible to ensure that all medications and refills are in place and ready for administration
  • Supervise menu planning and grocery shopping to ensure meals follow the guidance of EO509 healthy diet
  • Responsible for turning in all petty cash and receipts and required paperwork to the office in a timely manner, under the supervision of the PD
  • Monitor, maintain and reconcile all individual financial records and forward to guardians when appropriate
  • Forward all required material/paperwork to DDS, day programs, families & other government agencies under the direction of the PD
  • Provide a clean, safe and attractive program for the individuals
  • Provide direct care and facilitate house meetings
  • In conjunction with the PD, establish and maintain ongoing family contact
  • Organize and implement group and individual activities which serve to expand the lives of individuals and foster greater degrees of independence, under the direction of the PD.
  • To perform other related work duties as needed or as assigned by the PD

The following duties can be assigned to an APD (after 2 years in the position) provided all the following criteria are met:

  • APD desires the training and additional responsibility
  • The PD advocates that the APD could successfully perform the additional duties based on previous evaluations & supervisions
  • The President and Clinical Director approve of the increase in duties

Additional Duties:

  • Ongoing training and supervision of staff
  • Assists in the hiring and evaluation of all staff, including relief
  • Assists the PD to complete Medication Treatment Plans, ISP assessments, determine ISP goals, develop PSS and data forms and forward to DDS within 10 days of the ISP meeting
  • Represent their programs and/or individuals when dealing with DDS Service Coordinators and Office of Quality Enhancement as well as other state agencies

Qualifications

  • BA or BS in Human Service discipline preferred OR 6 years’ experience in related field.
  • At least 3 years of increasingly more responsible positions with a human service agency
  • Supervisory experience
  • Excellent written, verbal, and interpersonal skills.
  • Must possess a working knowledge of ISP process and Survey & Certification standards.
  • Proficiency utilizing Microsoft Office, video conferencing and the internet
  • Manages multiple priorities and resolves issues in a calm and pro-active manner
  • Work cooperatively and effectively with others to set goals, resolve problems, and make decisions that enhance organizational effectiveness
  • Valid Driver’s License
  • Successful completion of all state and agency mandated trainings including Medication Administration, CPR, First Aid
  • Must be willing to authorize a Criminal History Background Check (CORI), and Fingerprinting check through National Criminal Background Check (NCBC) and release of information concerning allegations from Disabled Persons Protection Commission (DPPC), Department of Developmental Services (DDS), and other agencies

Language Skills

Must be fluent in English and have the ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to communicate effectively with individuals under supervision, employees of the organization, families, and members of the community.

  • The President may waive these requirements.
  • This position corresponds with UFR Title description 133 – Direct Care/Program Staff Supervisor
  • Hours may be altered to meet the needs of the program. Ten calendar days’ notice given.
