Under the supervision of the Program Manager of Community and Family Wellness, the Employment Navigator will work with clients directly and often with family and health case workers. The goal is to achieve the best outcomes for each client in their well-being. In addition, the Employment Navigator will support projects, programs, and initiatives as assigned by the Program Manager. The Employment Navigator will work out of the Jurupa Valley Resource Center.

ESSENTIAL DUTIES/RESPONSIBILITIES
- Conduct assessments and partner with individuals to develop an individualized care plan based on information collected through assessments and interviews to address medical, social, educational, and other services needed to further goals and/or develop an action plan to remove barriers to success.
- Coordinate referrals to services and related agencies to meet the needs identified in the care plan. Assist with identifying and establishing natural and professional support in the community.
- Conduct fieldwork, including door-to-door engagement, cold calling, and meeting with potential employers.
- Establish regular contact with clients, carrying a caseload to monitor and assist progress toward goals with at least bi-monthly follow-ups.
- Facilitate job training and skill development programs, offering on-going support, mentorship, and workplace navigation assistance.
- Develop and maintain partnerships with local employers and businesses to create job placements tailored to the needs of formerly incarcerated individuals.
- Build and leverage community support networks, collaborating with community organizations, faith-based institutions, and other stakeholders.
- Stay flexible, professional, and available to support the clients at the allocated time and on a daily or walk-in basis.
- Facilitate group sessions and workshops in restorative and developmental support such as life skills, parenting, financial literacy, job readiness, substance use prevention, and health and wellness as an internal service provider.
- Monitor and document client progress in electronic systems or other designated databases, ensuring accuracy and confidentiality.
- Provide crisis intervention and support as needed and make appropriate referrals to emergency services.
- Coordinate program evaluation and quality improvement activities to ensure that services are meeting the needs of clients and are aligned with program goals and objectives.
- Maintain a safe and welcoming environment for clients, visitors, and staff by adhering to established policies and procedures related to safety and security.
- Carry out mandated reporting when necessary, or consultation when required.
- Accomplish clients’ care by assessing needs, developing, monitoring, and evaluating care plans and progress, including closing as needed and providing resources.
- Collaborate with community partners and stakeholders to identify gaps in services and resources, and advocate for needed changes.
- Support outreach activities to promote awareness of the resource center and its services to potential clients and community members.
- Coordinate any pertinent case management trainings for staff, to help develop best strategies when providing case management support
- Coordinate regular team meetings with staff to review client progress, discuss challenges, and identify opportunities for improvement.
- Attend training sessions and professional development opportunities to enhance knowledge and skills related to case management, community resources, and best practices in working with diverse populations.
Other Requirements
- Continue with regular assigned duties as determined by Director.
- Ability to work flexible schedule including nights and weekends.
- Perform other job-related duties as assigned.
- Provide translation support for documents and verbal communications if fluent in Spanish

Qualifications
Required Education: Bachelor’s degree in the following areas MSW, MSP, MPH or another related field.
Required Experience: A minimum of four-five (4-5) years’ experience in counseling and/or case management.
Preferred Experience:
- At least seven (7) years’ experience in counseling, case management, or related experience.
- Fluent Bilingual Spanish/English.
