Position Title: Urgent Care Advanced Practice Provider

REACH Medical seeks a full time Urgent Care Advanced Practice Provider. The Advanced Practice Provider is responsible to be the daily on-site Clinical Provider
who sees walk-ins, same day patients, covers the schedule for absent providers, and oversees daily coverage of prescriptive and clinical needs that fall outside of the daily provider schedule.

Responsibilities:

• Operate in an extremely flexible manner that allows for many patients to be seenper hour
• Maintain positive and professional interactions with patients, staff and contractors
• Collaborate with the Operations team supervisors and staff to see on average 3 patients per hour but be willing to see up to 4 patients per hour on a demanding day
• Conduct work at the REACH office in order to see either in person or via TeleMedicine patients from multiple portals to include:
  • New patients for Medications for Addiction Therapy (MAT)
  • Walk in and or Outreach patients
  • Patients who have missed appointments and need follow up MAT
  • Patients with acute primary care issues to include infections, skin abscesses, or minor injuries
  • Covering patient schedules when other clinical providers are out
  • Daily signing of prescriptions that are missed or need changes to include:
    • Medication refills
    • Bridges for missed appointments
    • Pharmacy changes
    • Prior authorization prescription changes

Qualifications

Qualifications:

• Licensed NYS Nurse Practitioner or Physician’s Assistant
• Demonstrated ability to work in a fast paced clinical setting
• Experience or a strong interest in delivering low threshold harm reduction care for individuals who use drugs
• Demonstrated knowledge and ability to work in a diverse and inclusive health care setting
• Ability to quickly learn cloud based Electronic Medical Record Systems

Benefits:

• This position is a 40 hour per week and is a benefits-eligible
• The salary scale for this position is $65-70 per hour
