Position Overview:
We are seeking a compassionate and dedicated Social Work Case Manager to join our team. The Case Manager will play a crucial role in providing comprehensive case management services to individuals and families in need. This role involves assessing clients' needs, developing individualized service plans, coordinating resources, and advocating for the well-being of clients. The ideal candidate will have strong interpersonal skills, a deep understanding of social work principles, and a commitment to empowering clients.
Key Responsibilities:
Client Assessment:
- Conduct comprehensive assessments to understand clients' physical, emotional, and social needs.
- Identify immediate and long-term challenges and opportunities for support.
Service Planning:
- Develop and implement individualized service plans in collaboration with clients.
- Set realistic goals and timelines, ensuring that clients are active participants in their care.
Resource Coordination:
- Connect clients with appropriate resources, including healthcare, housing, employment, education, and community services.
- Monitor and evaluate the effectiveness of services and make adjustments as needed.
Advocacy and Support:
- Advocate for clients' rights and needs within various systems, including healthcare, legal, and social services.
- Provide emotional support and crisis intervention as necessary.
Documentation and Reporting:
- Maintain accurate and up-to-date records of client interactions, service plans, and progress.
- Prepare reports and documentation required for compliance and funding purposes.
Collaboration:
- Work closely with other social work professionals, healthcare providers, community organizations, and government agencies.
- Participate in multidisciplinary team meetings to ensure coordinated care.
Education and Outreach:
- Educate clients and their families about available services and resources.
- Conduct community outreach to raise awareness about social services and programs.
Qualifications:
- Bachelor’s degree in Social Work (BSW) preferred but not required.
- Experience in case management or related social work is a plus.
- Strong knowledge of social services, community resources, and relevant legislation.
- Excellent communication, organizational, and problem-solving skills.
- Ability to work independently and as part of a team.
- Cultural competence and sensitivity to diverse populations.
- Proficiency in case management software and Microsoft Office Suite.
Working Conditions:
- Full-time position with potential for occasional evening or weekend hours.
- Office-based with some travel to clients' homes, community centers, and other service locations.
