Psychiatric Nurse Practitioner for Short Term Residential Therapeutic Program (IN PERSON)
Reach to Win Youth Home and Family Services is a Short Term Residential Therapeutic Program (STRTP) facility located in Antioch that provides residential and mental health services to girls ages 12 to 17 years old. We are seeking a dedicated and compassionate Psychiatric-Mental Health Nurse Practitioner to join our team in a contract role. In this role, you will provide comprehensive mental health care to foster teen girls, focusing on assessment, diagnosis, treatment, and management of psychiatric disorders.
You will be supervised by a Psychiatrist and work collaboratively with a multidisciplinary team to ensure the highest quality of care for our residents. This is an in person/hybrid position.
PRIMARY RESPONSIBILITIES:
- Conduct thorough assessments of patients’ mental health status and medical history.
- Develop and implement individualized treatment plans based on patient needs.
- Administer medications and monitor their effects on patients.
- Maintain accurate medical records and documentation in compliance with County and State of California healthcare regulations.
- Provide education and support to residents/clients and their families regarding mental health conditions and treatment options.
- Collaborate with primary care providers to integrate behavioral health into overall patient care.
- Attend virtual care coordination meetings
- Address urgent patient issues that arise outside of visits
- Thorough knowledge of HIPAA regulations and medical ethics as they relate to patient care
MINIMUM QUALIFICATIONS:
- A minimum of 2 years' experience in mental health programs with adolescents
EXPERIENCE:
- The ideal candidate will possess the following skills and experience:
- Proficiency in medical records management and documentation practices.
- Experience in medication administration, including knowledge of psychotropic medications.
- Background in mental and behavioral health with trauma informed adolescents.
- Familiarity with electronic health record systems.
- Experience administering medication
- Strong communication skills with the ability to build rapport with patients from diverse backgrounds.

Pay: $110.00 - $115.00 per hour
