**Job Summary:**
We are seeking a compassionate and dedicated Registered Nurse to join our Home Health team. The ideal candidate will provide high-quality care to patients in the comfort of their homes.
**Responsibilities:**
- Conduct in-home patient assessments and develop individualized care plans
- Provide skilled nursing care to patients in accordance with physician orders
- Monitor patient vital signs and symptoms, and report any changes to the healthcare team
- Educate patients and their families on disease management and treatment plans
- Coordinate with other healthcare professionals to ensure comprehensive patient care
- Maintain accurate and up-to-date medical records
**Qualifications:**
- Valid RN license in the state of practice
- Minimum of 1 year of nursing experience, preferably in home health or a related field
- Experience in pediatric care, case management, utilization management, infectious disease care, acute care, managed care, or critical care is a plus
- Strong communication and interpersonal skills
- Ability to work independently and as part of a multidisciplinary team
- Proficiency in medical documentation and electronic health records
