Team Lead Job Description
Summary/Objective
The Team Lead is a part of the Care Team and performs a variety of duties that relate to managing client care on a professional level. The Team Lead works closely with the Central Scheduler to advise as to where to schedule staff in order to coordinate in home care to patients with mild to chronic conditions. Through care coordination and integration, the Team Lead is responsible for assuring access to appropriate services, improves health outcomes, reduces preventable hospitalizations and emergency room visits, promotes use of health information technology and avoid unnecessary care.
This position carries out responsibilities in the following functional areas: client management, medication management, employee management, policy compliance, and record keeping.
Essential Functions

1. Reflect the core values of ReadyCare
2. Meet with clients and family members to discuss their needs and provide solutions in the form of a service/care plan.
3. Work with other team members to coordinate various aspects of a client’s care.
4. Maintain client and responsible party records including resident specific timecards.
5. Recognize and pursue opportunities to modify service plans to best support the ongoing needs of client, with the approval of the Clinical Manager.
6. Demonstrate open and effective communication with clients, family members, the Central Scheduler,
Regional Manager, colleagues and caregivers, referral providers/care providers, and the owners.
7. Work with clients and their families on the various issues that may arise to ensure they are getting resolution to issue as well as receiving the services needed.
8. Work with the pharmacy to ensure all necessary medication are delivered and administered as ordered.
9. Administer Med Tech Training to qualified candidates and monitor as necessary per Med Tech manual.
10. Manage floor staff, including daily rounds communication via email.
11. Maintain regular attendance to execute job responsibilities.
12. Adhere to all company policies, procedures and business ethic codes and ensures that they are communicated and implemented within the team.
13. Rotate Emergency On-Call with the Central Scheduling Department, 1 weekend per month.
Secondary Functions
1. Conduct all caregiver meetings.
2. Conduct family Education sessions as needed.
3. Perform any and all other functions as deemed necessary by the Regional Manager.

Supervisory Responsibility
This position has direct supervisory responsibilities and serves as a coach and mentor for other positions in the sub class field department.
Work Environment
This job operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.
Physical Demands
The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
While performing the duties of this job, the employee is regularly required to talk or hear. This is largely a sedentary role; however, some filing is required. This would require the ability to lift files, open filing cabinets and bend or stand on a stool as necessary.
Position Type/Expected Hours of Work
This is a full-time position. Days and hours of work are typically Monday through Friday; but may be structured according to the needs of the community. Occasional evening and weekend work may be required as job duties demand, as well as a rotation of Emergency On-Call with the Central Scheduler one weekend per month. When On-Call, any open shift must be restaffed by the Team Lead or worked by the Team Lead if unable to find relief. Travel while On-Call is prohibited.
