Job Title: Care Manager/Admissions Nurse
Job Summary:
As a Care Manager/Admissions Nurse, you will be responsible for assessing patient needs, coordinating care plans, and facilitating admissions to healthcare facilities. You will work closely with patients, families, and healthcare providers to ensure the highest level of care and support.
Responsibilities and Duties:
- Conducting assessments to determine patient needs and develop individualized care plans
- Coordinating care and services with healthcare providers and support staff
- Facilitating admissions to healthcare facilities and coordinating transitions of care
- Providing education and support to patients and families
- Monitoring patient progress and making necessary updates to care plans
- Collaborating with interdisciplinary teams to ensure seamless care delivery
- Documenting patient information and ensuring compliance with regulatory requirements
Qualifications and Skills:
- Valid RN license and BSN preferred
- Minimum of 2 years of nursing experience, preferably in care management or admissions
- Strong assessment and critical thinking skills
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Knowledge of healthcare regulations and quality standards
