Job Title : Case Manager – Outpatient Mental Health Day Program

Reports To : Program Director / Clinical Supervisor

Position Summary :

The Case Manager plays a pivotal role in supporting clients enrolled in our outpatient day program. This full-time position (Monday through Friday, 8 : 00 AM to 4 : 00 PM) involves comprehensive case management—from intake assessment to the development and implementation of individualized treatment plans. The successful candidate will work collaboratively with clients, families, and a multidisciplinary team to ensure that each client receives the tailored support they need to achieve mental health and recovery goals.

Key Responsibilities :

Client Assessment & Treatment Planning :

Conduct thorough intake assessments to understand each client’s mental health status, history, and current needs.

  • Collaborate with clients and clinical staff to develop, implement, and periodically review individualized treatment plans.

Care Coordination & Service Management :

Coordinate referrals to internal programs and community resources to support clients’ comprehensive care needs.

  • Schedule and manage client appointments, ensuring continuity of care and timely follow-up.
  • Liaison & Advocacy :

    Act as a liaison between clients, their families, and external service providers, ensuring open communication and coordinated care.

  • Advocate for clients by identifying barriers to care and developing strategies to overcome them.
  • Documentation & Compliance :

    Maintain accurate, confidential, and up-to-date records of client interactions, treatment plans, and progress notes in compliance with HIPAA and facility policies.

  • Prepare reports and participate in audits or reviews as required.
  • Team Collaboration & Professional Development :

    Participate in regular team meetings to discuss client progress, share insights, and refine program strategies.

  • Engage in ongoing professional development and training to remain current on best practices in mental health care and case management.
  • Qualifications :

    Educational Background :

    Bachelor’s degree in Social Work, Psychology, Counseling, or a related field is preferred but not required

    Experience & Skills :

    Demonstrated experience in case management or mental health services, ideally within an outpatient or community-based setting.

  • Strong knowledge of mental health disorders, treatment modalities, and crisis intervention techniques.
  • Excellent communication, interpersonal, and organizational skills.
  • Proficiency with electronic health records (EHR) and standard office software.
  • Certifications :

    Relevant licensure or certification (e.g., LCSW, LPC) is preferred, where applicable but not required
