Job Description:
- Asses each resident’s skin condition weekly and as necessary including
stage, size, sites, depth, color, drainage, and odor of each condition. Report any changes to physician,
charge nurse, Nursing Supervisor, Director of Nursing, and take follow up
action.
- Review and update residents care plan as
necessary.
- Observe infection control procedures.
- Instruct nursing assistants in proper,
preventative safety measures and use of adaptive equipment to meet residents’
needs.
- Participate in interdisciplinary team and
rehabilitation meetings as required.
- Maintain confidentiality of resident and
facility information.
- Perform other related duties as directed by supervisor.
Skills and Qualifications:
- Valid LVN License
- Valid CPR Certification
- Wound Certification is plus
- Excellent verbal, written and interpersonal communication skills
- Excellent attention to detail
- Ability to carefully read and follow directions
- Good teamwork skills
- Ability to lift and move patients
