Position: Care Coordinator
Job Type: Full-Time / Non-Exempt
Shift Details: Rotating - 7:00AM - 7:30PM
Work Location: Crisis - Franklin County Crisis Care Center; Columbus, OH
Salary/Hourly Wage: $19.00 - $22.00/hour
Specialties / Licenses or Certifications:
- Bachelor’s Degree in a behavioral health field and a minimum of one (1) year of behavioral health experience; or
- Bachelor’s Degree in any field and a minimum of two (2) years of behavioral health experience; or
- High School Diploma or equivalent and a minimum of four (4) years of behavioral health experience.
Summary of Job: The Care Coordinator facilitates the quality and improvement of a person’s experience of care, which may include admission, discharge, outreach, engagement, recovery, transition and case management.
Essential Duties and Responsibilities:
- Guest Care & Advocacy:
- Provides coordination of care of services and resources available internally as well as throughout the community.
- Acts as an advocate for the needs and rights of individuals.
- Assists individuals in daily living activities and establishing and maintaining linkages that provide education, support and independence in the community.
- Compliance & Risk Management:
- Documents relevant and required information and services assuring coordination of care as a tool for communication with other team members.
- Keeps abreast of all federal and state regulations and laws and all Recovery Innovations policies and procedures as they presently exist and as they change or are modified.
- Understands and adheres to Recovery Innovations compliance standards as they appear in the Code of Conduct and Conflict of Interest Policies.
- Cross Functional Collaboration:
- Coordinates or facilitates multidisciplinary team meetings to ensure services are provided in a timely manner.
- Partners with other provider agencies in the coordination of care to help participants achieve wellness.
- Additional Duties:
- Performs other related duties as assigned.

- Education/Licenses:

- Driving Requirements:
- If driving is required for this position, must meet all requirements outlined in Recovery Innovations driving policy.
- If not qualified to drive per driving policy, a Non-Driver Attestation form must be signed.
- Miscellaneous Requirement(s):
- Travel required based on business need.
- Applicants must be at least 18 years old unless residing in a state where funder or government regulations set a different minimum age requirement.
- Ability to maintain a high level of confidentiality (HIPAA guidelines and regulations) and professionalism.

Physical Demands:
May include work close to the eyes and substantial motions of the wrist, hands, and/or fingers. Frequent sitting, standing, walking, handling, finger dexterity; eye-hand-foot coordination, use of corrected vision; color vision; use of auditory senses. May be required to lift up to twenty (20) pounds.
Work Environment
May work in an office environment or behavioral health facility. May have contact with individuals who are agitated and/or confused.
