Job Title: Case Manager: The Point Cafe (POWR)
Summary/objective: The Cafe Case Manager, on the POWR (Providing Oppurtunity for Womens Recovery) grant, is responsible for providing comprehensive case management services to people with a history of SUD/COD. Working out of the Point Cafe, a community recovery resource center, the Case Manager will advocate for women and their families and provide assistance with linking and referring them to community resources based upon their identified needs and established care plans.
Essential functions: Reasonable accommodations may be made to enable individuals with disabilities to perform these essential functions.
- Work with clients to develop individualized case management plans.
- Continually monitor and evaluate each client’s progression through their case management plan and develop corrective action revisions to the plan as needed.
- Assist clients in obtaining necessary resources to achieve case management goals.
- Actively participate in client and program-related meetings and trainings.
- Attend program-related community, coalition and community meetings as assigned.
- Prepare case management related reports including but not limited to outcomes, successes, etc.
- Conducts intake reviews/assessments including GPRA.
- Identify and connect with resources within the community.
- Other duties as assigned
Education/Qualifications: Bachelor’s degree in Social Work or Allied Health Field is required. At least one (1) year work experience in the field of addiction or related field preferred. Valid driver’s license and access to transportation required.
Knowledge, Skills, Abilities: Must have excellent verbal and written communication skills, basic computer skills is required.
