Forensic Case Manager
Forensic case management uses a treatment focused approach that collaborates with criminal justice systems to stabilize mental health symptoms and reduce recidivism. The Forensic Case Manager is an integral member of a client-centered team that provides intensive community-based services. Forensic Case Managers use a supportive and recovery-oriented approach to treatment and strongly support clients in successfully integrating within their community.
In this role, you’ll
- coordinate with other professionals including parole officers, probation officers, court staff, and forensic liaisons
- refer and link clients to the Agency and other community resources to address basic needs such as food, clothing, housing, and benefits
- coordinate treatment planning to identify goals
- utilize evidence-based strategies to support mental health recovery
- remove barriers that prevent access to care
- monitor and facilitation of addressing need across the healthcare spectrum to include physical health, mental health, and other specialty healthcare needs
- work with clients with high demand

Qualifications
Minimum qualifications include
- High school diploma (or equivalent) and three years of direct case management experience, or in lieu a bachelor’s degree in a related field
- Good interpersonal, written and verbal communication skills
- Understand mental illness, symptoms and impact on behaviors; understand concept of recovery/resilience
- Have an ability to think critically in a fast-paced environment and manage time effectively.
- Be comfortable transporting clients (valid driver license, vehicle, and insurance a must)
