Reintegration Case Manager
Reintegration case management uses a treatment focused approach that collaborates with criminal justice systems to stabilize mental health symptoms and reduce recidivism. The Case Manager uses a client-centered approach that provides intensive services while the client is still incarcerated and continues when the client is released back into the community.
In this role, youll
coordinate with other professionals including parole officers, probation officers, court staff, and forensic liaisons
refer and link clients to the Agency and other community resources to address basic needs such as food, clothing, housing, and benefits
coordinate treatment planning to identify goals
utilize evidence-based strategies to support mental health recovery
remove barriers that prevent access to care
monitor and facilitation of addressing need across the healthcare spectrum to include physical health, mental health, and other specialty healthcare needs
work with clients with high demand

High school diploma (or equivalent) and three years of direct case management experience, or in lieu a bachelors degree in a related field
Good interpersonal, written and verbal communication skills
Understand mental illness, symptoms and impact on behaviors; understand concept of recovery/resilience
Have an ability to think critically in a fast-paced environment and manage time effectively.
Be comfortable transporting clients (valid driver license, vehicle, and insurance a must)
