Mental health care is often overlooked, yet essential for those who need it most. As a Recovery Plan Coordinator at Recovery Solutions, you will have the opportunity to provide quality care to every patient with compassion, collaboration, and innovation.
The Recovery Plan Coordinator plays a crucial role in organizing, coordinating, facilitating, and monitoring the provision of all treatment and rehabilitative services for patients in assigned patient living areas.
Your primary goal is to help patients acquire and use the skills and supports necessary to achieve maximum independence, success, and satisfaction in their preferred environment.
This includes initiating and maintaining Recovery Plans in accordance with all facility policies, protocols, and guidelines.
You will coordinate and facilitate the development of Recovery Plans and associated documentation, and schedule Treatment Team meetings to review and update plans on a monthly basis.
Advocate for and make referrals for individualized services based on assessed need, monitor patient participation and progress, and conduct monthly visitation reviews.
Serve as an expert resource on psychiatric rehabilitation, provide training to team members, and demonstrate cultural and age-specific competency in all duties performed.
